I have received a letter from Access to Health and Social Care

The Business Services Organisation (BSO) maintains a list of patients registered with a GP in
Northern Ireland. The BSO must verify the information on this list to ensure it is accurate and up
to date and that the patients listed are entitled to receive publicly funded health care in Northern
Ireland. To do this the BSO screens patient data they hold against date held by other day to day
service providers.

As we have been unable to verify the information we hold for you. We are writing to advise you of
this and request that you complete and return the enclosed questionnaire. This is required in
order that we can verify our information and assess your entitlement to access publicly funded
health care in Northern Ireland.
